CMS and the OIG have identified the practice of copy/paste (copying and pasting previously recorded information from a prior note into a new note) as a problem in healthcare institutions that has yet to be broadly addressed.

The timesaving practice of cloning eliminates the need to re-document every part of a note that is pertinent but remains unchanged, or interview patients about their medical, family, and social histories at each visit. But regular cloning of clinical information from unique patient encounters can lead to an increase in billing at a higher level of service than was actually provided. Yet it is estimated that 74-90 percent of providers use the copy/paste function in their EHRs and between 20-78 of physician notes are copied text, according to an AHIMA report issued in September 2016.

  • Use of smart text/smart phrases, templates, auto-fill, auto-prompts, and copy/paste
