Webinar/Audio Conference FAQs
Failure to use the correct HCPCS Level II modifier may result in claims denial. Nancy will discuss how modifiers (or lack thereof) cause denials and why.
Many modifiers are considered “informational only” and do not affect the processing of the claim or reimbursement. In some instances, insurers tell providers that a HCPCS code must be accompanied by a code modifier to provide additional information regarding the service or item identified by the HCPCS code. Modifiers are used when the information provided by a HCPCS code descriptor needs to be supplemented to identify specific circumstances that may apply to an item or service.
Medicare coverage status for the Level II modifier:
The codes and descriptions for these alpha characters include:
| D | Special coverage instructions apply |
| I | Not payable by Medicare (no grace period) |
| G | Not payable by Medicare (90-day grace period) |
| M | Non-covered by Medicare |
| S | Non-covered by Medicare statute |
| C | Carrier Judgment |
Other Modifiers Discussed:
- Specialized Modifiers
- Coronary Artery Modifiers for Interventional Cardiology Procedures
- Rt and Lt Modifiers
