To prepare for ICD-10, Physicians and other providers must learn to document for medical necessity. The new codes for 2010 gives us an example of what is to come in the new ICD-10 coding documentation that is currently scheduled for 2013. If your provider does not document well with Version 9, they will be unprepared for the conversion to Version 10.
