Medical record documentation too often fails to meet the medical necessity required to support the appropriate level service. Learn to simplify the complexities of medical necessity and understand how to properly document the medical record to support it. Has the correct code been selected? Is it properly linked from progress note to billing record? Are the services provided medically necessary at the frequency they have been provided? Are they medically necessary for the diagnosis or treatment of the specific illness or injury?

Clarify misconceptions about Documentation Guidelines with a walk through Evaluation and Management Services as they relate to medical necessity.

Gain insights to help you avoid common errors that may trigger an audit. Have your providers fallen into the trap of relying on EHR templates? Government and commercial third party payers regularly use data mining to identify when patterns and/or trends exist in the medical record that may indicate potential fraud, waste, or abuse. When no documentation differences are noted for several services for one beneficiary or for services for multiple beneficiaries, not only is medical necessity called into questioned but there may be a question of potential fraud.

About the Presenter

Karen Scott has a career that spans both healthcare and education. As a healthcare business manager, her career encompassed reception, medical records, coding, billing, collections, negotiation, payroll, credentialing, and all phases of the day to day operations of a busy multi-specialty medical practice. Karen has a passion for lifelong education and has taught adult learners for over 30 years at community colleges in Washington state, where she received the Washington Award for Vocational Excellence in 2000. She has developed medical billing and coding programs for two community colleges and has extensive experience in online course development and facilitation.
