Increase your knowledge of proper billing practices for Evaluation and Management (E/M) services. E/M error rates have historically been high. Does your office have a plan to improve E/M error rates in EHRs?

It is inappropriate to bill a higher level when a lower level code is warranted. While copying and pasting data is not always a negative, the data must be reviewed for accuracy. You must have protocols in place that stipulate a provider must review any imported data, make necessary corrections, and add their signature attesting that they reviewed the imported data and found it to be correct or made necessary modifications. Your policy should also stipulate the chief complaint must be compared to the treatment plan and all issues must be addressed.

Providers are responsible for billing the appropriate E/M code to Medicare. CMS encourage its contractors to review billing for E/M services and contracts with Recovery Audit Contractors (RAC) to conduct postpayment reviews to identify and correct improper payments. Another contractor produces comparative billing reports, which show a physician’s billing pattern for various procedures or services and compares that physician’s billing pattern to those of his or her peers. Regina will show you how to bill appropriately and protect your practice from undue risks in this audit-intensive healthcare climate.
