Frequently we come across claims where modifiers have been applied to override a National Correct Coding Initiative (NCCI/CCI) edit and allow commonly bundled services to become unbundled and individually paid. The application of modifiers -24, -25, and -57 to override an edit when combined with a surgical procedure is commonly seen. Yet, too often the documentation to support such an override is seriously lacking. The same goes for overriding an edit to unbundle multiple, bundled surgical procedures performed during the same operative session by application of modifier 59 or one of the Medicare X{EPSU} modifiers.

About the Presenter
