The Physician Fee Schedule Final Rule for 2026 codified two conversion factors, an efficiency adjustment, and a reduction in work RVUs for non-time-based services. CMS has recalibrated Practice Expense (PE) methodology, updated Telehealth and supervision flexibilities, and introduced new add-on codes for Advanced Primary Care Management (APCM) services. While the One Big Beautiful Bill Act (OBBBA) did not explicitly set physician payment rates, it impacted the financial landscape in which providers operate. Adjustments in federal funding for Medicaid and state programs affect how states reimburse and contract with providers. With roughly ten million individuals projected to lose coverage by 2034, practices that have previously relied on Medicaid and/or marketplace plans will need to prepare for rate adjustments, prior authorization changes, and shifting enrollment patterns.
